Eligibility Guidelines

Referrals are appropriate for individuals who may:

  • Be experiencing homelessness or housing instability

  • Have substance use or addiction-related needs

  • Experience complex health or social challenges

  • Require coordinated, multi-service support

  • Face barriers accessing traditional systems of care

Priority may be given to individuals with higher complexity or urgent stabilization needs.

Referral Considerations

The HART Hub complements existing community services and works collaboratively with partner organizations. Submission of a referral does not guarantee program acceptance or immediate service placement.
